Insurance

I accept the following insurance: Aetna, Cigna, UnitedHealthcare/Optum, Humana Dual (Medicare & Medicaid), Anthem, Humana, Kaiser Permanente (Medicare and Medicaid), Blue Cross Blue Shield.

Reduced Fee

Reduced fee services are available on a limited basis.

Payment

Cash is accepted for payment.

Cancellation Policy

Cancellations and rescheduled sessions will be subject to a $65 fee if not received at least 48 hours in advance. This fee will be charged to the card that is on file.

If there is cancellation or a no-show without any communication from the client for more than 24 hours, the full session amount will be applied to your account.
